Clinical pharmacists apply their pharmaceutical knowledge to clinical situations, assisting medical practioners with optimising medication selection, dosing and monitoring.
A clinical pharmacist is trained to identify medication related problems, detect prescribing cascades and therapeutic gaps in a patient's management, recommend changes to therapy to achieve therapeutic goals and improve a patient's quality of life.
Our primary clinical pharmacy service is a Home Medicines Review, which takes place in a patient's home and is provided by an accredited pharmacist on the referral by a General Practitioner or other Specialist Doctor.

Graduated from the Western Australian Institute of Technology in 1982 with a Bachelor of Pharmacy and a Graduate Diploma in Clinical Pharmacy in 2017 from the University of Tasmania, With over 30 years experience in community and hospital pharmacy, Domenic has been an accredited pharmacist conducting medication reviews since 2009. He is a member of the Pharmaceutical Society of Australia and an associate of the Australian Consultant Pharmacists Acssociation.
