For prescribers a Home Medication Review can:
reconcile the patient's medication list,
reduce the risk of polypharmacy,
improve patient awareness of their medicines,
provide insight into the patient's adherence to the prescribed medication regimen,
provide a better understanding of patient medication and lifestyle issues,
assist to determine if the patient has multiple prescribers,
ensure patient continuity of care.
If you feel that your patient will benefit from a Home medicines review then you can email your referral to pharmacist@clinicalpharmacy.com.au or click on the Send by email button or upload the HMR referral by using the Document Upload Form below.
An accredited pharmacist will;
contact your patient to arrange a suitable time to visit,
conduct the medication review, and
return the HMR report to you via email.
Please ensure that patient's contact details and your contact details are included on the referral.
