Need to find an accredited pharmacist to perform an HMR for one of your patients? We can help you.  

If you have received a Home Medicines Review referral from a patient and you need an accredited pharmacist to conduct the review, then you can either email the referral  to pharmacist@clinicalpharmacy.com.au or use the button below.

Alternatively you can use the document upload form below.

 If an accredited pharmacist has requested a patient's dispensing history, please email the document as an attachment to pharmacist@clinicalpharmacy.com.au or the use the Upload Documents  button below.

Please include your email address on the documents and we will let you know we have received them.  

A copy of the completed HMR will be sent to you. If we received a response from the Doctor, we will also send this to you.
